It’s a really exciting time to join us as we continue to deliver the Heathier You NHS Diabetes Prevention Programme throughout North East London and we’re looking for someone to join our team of passionate health coaches (or Diabetes Prevention Practitioners)!
The purpose of a health coach is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.
Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.
A typical day as a Diabetes Prevention Practitioner might include :
- Delivering face-to-face evidence-based interventions that focus on diet, nutrition, physical activity and behaviour change
- Facilitating group intervention sessions to support your clients in achieving their goals and cultivating a safe and positive environment
- Calling patients who have been referred into the programme to book them into upcoming groups
- Engaging in ongoing training relevant to your role, including submitting your annual CPD portfolio and evidencing professional development in line with our CPD policy
- Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

We’re looking for someone who can combine health improvement knowledge with first class communication and motivational skills. If you have experience of delivering group-based interventions, and a real understanding of the impact personal characteristics and circumstances can have on health behaviours then we want to hear from you. Bonus points if you have a qualification in a health promotion related subject, such as nutrition, exercise, or health coaching!
We hire based on attitude over traditional skills and experience so if you don’t have all of the necessary experience but feel you are right for the role we would love to hear from you!

Contract: 1 x Permanent, Full Time (37.5 hours) position available
Salary: £24,000 per year (rising to £27,000, with development opportunities to earn up to £31,000 as a Senior Health Coach)

Location: This is a hybrid-remote role. You will need to live in North East London as the role requires regular travel across North East London along with some working from home.
