This is an exciting role that will provide tailored specialist stop smoking support, over a 6-week intervention, to people living in Hammersmith & Fulham area. We work with the local primary care network to support clients who are looking to improve their Health & Wellbeing specifically with quitting smoking.
Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.
A typical day as a Specialist Stop Smoking Advisor might include:
- Delivering evidence-based 1:1 intervention that focus on smoking cessation, behaviour change, and psychological support to save lives by helping smokers quit for good.
- Running stop smoking clinics across H&F within the local health centres and medical centres, with a potential of running clinics in a community setting.
- Conducting CO-level testing, advising on pharmacotherapy and requesting prescriptions.
- Managing a caseload of clients and monitoring and recording outcomes in Microsoft SharePoint and SystmOne.
- Collaborating with practice managers, GPs, nurses, pharmacists and link workers within the PCN to help raise awareness about the service and improve patient uptake.
- Ensuring an excellent and safe experience for clients, in alignment with our policies and procedures.
- Engaging in training relevant to your role, such as NCSCT Stop Smoking training and Behaviour Change Level 1 and 2
- Co-producing with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments; for example, sharing ideas for creating new resources or patient feedback pathways– we’re always looking to share knowledge and resources!
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!
