At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier! We’re looking for an NHS Health Check Practitioner (2 roles) to join our One You East Sussex service.
Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.
A typical day as an NHS Health Check Practitioner might include:
Providing NHS Health Checks across East Sussex, in GP practices, workplaces and community venues.
To carry out your own outreach to source community and workplace health check clinics in East Sussex.
Being compassionate and empathetic when working with clients.
To register clients on our database, record measurements such as BP, BMI, finger prick blood sample for glucose and cholesterol and combine this with lifestyle questions.
To work onboard our mobile Health Check unit - COLIN!
To empower clients to improve their health, by motivating change within their lifestyle.
To sign post patients on to relevant local health and wellbeing services.
To develop and maintain effective working relations with key partners and other health professionals to establish appropriate referral systems and ensure holistic provision of care.
Promoting our services across East Sussex.
Working with colleagues across all our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

To work with us in this role you will need to have some delivery experience of supporting clients to make healthy lifestyle choices. However, we hire based on attitude over traditional skills and experience so if you feel you are right for the job we would love to hear from you!

Contract: 1 Role is a Fixed Term Role Up to March 2027 (Extension possible pending funding), this fixed term role is for 37.5 hours (FULL TIME)
1 role is a permanent PART TIME ROLE (15 or 22.5 hours per week)
Salary: £23,810 for the full time role. This will be pro rata for the part time role.

Location: Our main base is in Hampden Park, Eastbourne, however clinics will be across East Sussex.
