At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier! We’re looking for a highly motivated and driven Health Check Practitioner to join our service in East Sussex.

You will be responsible for the delivery of Health Checks to promote health and prevent long-term health conditions. This is a great opportunity to help people improve their health through the delivery of Health Checks and provision of motivational lifestyle advice.

A typical day as a Health Check Practitioner might include :

• Providing Health Checks across East Sussex.

• Being compassionate and empathetic when working with clients.

• Carrying out your own outreach to source some of your own health check clinics at workplaces and community venues across the county.

• Delivering NHS Health Checks to our clients, which includes carrying out a blood pressure check, a cholesterol and blood sugar test (finger prick blood sample), height and weight measurements and asking diet and lifestyle questions.

• To record client's measurements on our database and effectively explain their results at the end of the appointment, referring to their GP or our wider One You East Sussex service if necessary.

• To sign post patients on to relevant local health and wellbeing services.

• To empower clients to improve their health, by motivating change within their lifestyle.

• To work onboard (and drive!) our mobile Health Check unit - COLIN!

• To develop and maintain effective working relations with key partners and other health professionals to establish appropriate referral systems and ensure holistic provision of care.

• Promoting our services across East Sussex.

• Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!

• Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.

• Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

To work with us in this role you will need to have some delivery experience of supporting clients to make healthy lifestyle choices. Contract: Part time, 30 hours per week. Permanent.

Salary: £19,058 per year (with the potential to rise to £22,400 depending on future developments)

Location: This job is based in East Sussex, with our office base in Hampden Park, Eastbourne, East Sussex.

The job will involve some home working when not delivering clinics and will include travel across the whole of East Sussex.

Because of the travel requirements, being a car driver is essential for this role (travel expenses will be paid where necessary).

Please note, the successful applicant will be required to have the right to work in the UK and have a Full UK Driver’s License.
