It’s a super exciting time to join us as we begin to deliver Stop Smoking Services throughout County Durham, and we’re looking for a Primary Care Support Officer to join our team at Smokefree County Durham!

Our Stop Smoking Programmes are designed to support local people who live, study or work in County Durham and want, or need, to quit smoking. As a Primary Care Support Officer, your role will be to successfully engage with Primary Care partners and organisations, and support referral pathways from Primary Care into the service.

The Primary Care Support Officer aims to provide a dedicated point of contact in our organisation and further develop effective links between our SmokeFree County Durham service and primary and secondary care partners. Our focus is to forge strong relationships with primary care colleagues and continually explore ways to build and improve these relationships to ensure the best care for our clients.

A typical day as a Primary Care Support Officer might include:

• Forming, developing and maintaining relationships with Primary Care organisations, partners and individuals.

• Attending networking/ outreach events and board meetings to share information about the service, ensuring colleagues in primary and secondary care are aware of the eligibility criteria and referral routes.

• Working with Primary Care to ensure the service is promoted and marketed to a high standard.

• Arranging Practice visits and events and ensuring GP’s and practice teams are kept up to date with relevant news, including producing and distributing a designated primary care newsletter.

• Engage in joint up working with primary and secondary care to develop effective and efficient referral routes into the service.

• Using population health data to identify key priority groups and use this to target our service to key demographics and communities.

• Working together with primary care colleagues to create targeted messages/ letter to patients, whilst supporting their population health goals.

• To produce written reports evaluating engagement and service outcomes, identifying what has worked well and recommend any areas of improvement to appropriate leadership teams.

• Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments- we’re always looking to share knowledge and resources!

• Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance

• Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

To work with us in this role you will need to have some knowledge or experience in networking, referral generation and/ or partnership management, as well as an understanding of health inequalities and approaches on how to engage those experiencing them into health promotion services. It is important that you are able to navigate the primary care landscape, with an understanding of how patients access care with bonus points if you have worked in a primary care setting previously!

Contract: Full time (37.5 hours), Permanent

Salary: £26,000

Location: Your base to work from is our office at The Greenhouse, Annfield Plain, County Durham. Much of your time will be spent engaging with Primary Care face-to-face across the whole of County Durham (so a car owner is desired). When not delivering across the County, you will be able to work both from our office base and from home.
